$50K - $55K + Super

Established in February 1995, my client is a major primary health care provider in the Latrobe Valley and Gippsland area.

My client has an integrated and co-ordinated approach to the delivery of services and is employed in community settings at local, sub regional and regional levels. This is achieved through inter-sectoral and inter-agency partnerships. 

Due to rapid growth within the Community an opportunity has arisen for an experienced qualified Support Coordinator to join their Warragul Team.

To be successful within this role you must have:

  • Strong knowledge of case management principles and the ability to manage through brokerage.
  • High level communication, negotiation and conflict resolution skills.
  • A demonstrated understanding of the issues and concerns of a variety of people.
  • Proven ability to liaise with a variety of service providers, professionals, caregivers and internal and external stakeholders.
  • Demonstrated understanding of Continuous Quality Improvement and Risk Management principles.
  • Ability to work autonomously and priortise tasks to ensure key objectives of the program are achieved.

Mandatory Requirements,

  • University level qualifications in a related discipline (Minimum Certificate IIII in Aged Care).
  • A current Victorian Drivers Licence.
